How to request accommodations

Follow these steps to request accommodations for courses, residencies or other university activities:

  • Complete and submit the Accommodations for Learners with Disabilities request form or contact Disability Services to discuss your needs.
  • Determining reasonable accommodations is an interactive process. If you have documentation on hand, please submit it to Disability Services for your initial consultation. Disability Services will reach out to you to discuss your request.
  • You may be required to submit supporting documentation from an appropriate professional, such as a medical doctor, psychologist and/or psychiatrist. Documentation should be current, on physician letterhead and contain the diagnosis of disability, functional limitations and recommendations for academic adjustments.
  • If accommodations are approved, you will be sent a Letter of Eligibility for Accommodation.
  • At the beginning of each course be sure to share your accommodation letter with your instructor(s), preferably through courseroom mail.

Please submit the Copy of Accommodation Letter Request Form to request that a copy of your letter be emailed to you.
