How to request accommodations
Follow these steps to request accommodations for courses, residencies or other university activities:
- Complete and submit the Accommodations for Learners with Disabilities request form or contact Disability Services to discuss your needs.
- Determining reasonable accommodations is an interactive process. If you have documentation on hand, please submit it to Disability Services for your initial consultation. Disability Services will reach out to you to discuss your request.
- You may be required to submit supporting documentation from an appropriate professional, such as a medical doctor, psychologist and/or psychiatrist. Documentation should be current, on physician letterhead and contain the diagnosis of disability, functional limitations and recommendations for academic adjustments.
- If accommodations are approved, you will be sent a Letter of Eligibility for Accommodation.
- At the beginning of each course be sure to share your accommodation letter with your instructor(s), preferably through courseroom mail.

Disability frequently asked questions
An individual with a disability is defined as a person who has a physical or mental impairment that substantially limits a major life activity, has a record of such an impairment or is regarded as having such an impairment.
It is recommended to have professional documentation of your disability prior to discussing your accommodation needs. However, if you do not have documentation readily available, please do not hesitate to contact Disability Services, as we can begin the accommodation process without it.
Documentation can take a variety of forms, including IEP/504 plans, letters from doctors/psychologists, accommodation letters from previous schools, hospital reports, etc.
Capella University respects your independence, rights and dignity. Therefore, identifying yourself and requesting accommodations are completely voluntary. In accordance with the Family Education Rights and Privacy Act (FERPA), Capella University handles all disability-related information confidentially, with the potential for disclosure only on a need-to-know basis.

- Barriers that may be encountered and the array of accommodations that might remove them
- Whether the learner has access to the course, program, service, activity or facility without accommodations
